Single Play Mode
Story Buff
Clear story mode with any character. Any difficult.
You can change the number of rounds in Options > Game settings, then setup to 1 round (default is 2) to make Story Mode faster.

Story Devotee
Clear story mode on hard with any character using no continues and with a round count of 3.
You need to go Options > Game Settings, then change the number of rounds to 3 (default is 2).
High-Scorer
Achieve Rank S in Score Attack Mode on any difficulty with any character.
Score Attack consist in 10 matches, 2 rounds to win.
Time Attacker
Achieve Rank S in Time Attack Mode on any difficulty with any character.
Time Attack consist in 10 matches, 2 rounds to win. Less of 15 min (normal) do the trick.
Survivor
Achieve Rank S in Survival Mode on any difficulty with any character.
In this mode you fight versus a “unlimited” amount of enemies. Fights are 1 round each, you heal back a little between rounds. So I advice you abuse from the Heat Mode mechanic (active when having at least 1 Magic Circuit bar by pressing A+B+C) to Heal up a little more. The minimum of wins to achieve S rank is 20 wins.
Perfect Clear Rate
Get a Clear Rate of 100% in Single Play Mode using any character.
For this you need:
– Complete Story mode in Easy, Normal and Hard,
– Rank S in Score Attack mode (Rank S in Easy, Normal and Hard),
– Rank S in Time Attack (Rank S in Easy, Normal and Hard)
– Rank S in Survival (Rank S in Easy, Normal and Hard)
– All missions (Special Challenge, Beginner, Intermediate and Expert Combo Challenges)
– All records (voices, illustrations, icons, plates and titles earned by achivement).
You can check progress in Single play > Character > Achivements.
Like a Long-Forgotten Dream
View any story in Memory.
Once you have beat Story mode for a character.
Single play > Select a character > Memory
Boss Rush completed!
Completed the Boss Rush story.
Once you have complete all character story, this mode will unlock with a “special arc”. You need to beat the Story mode with Miyako Arima. Free mode doesnt count (Thanks to @Saikyo for the confirmation).
You can change the number of rounds in Options > Game settings, then setup to 1 round (default is 2) to make Boss Rush easier. Not sure if the Difficult option affects (needs confirmation).
This mode has 7 fights vs Boss Characters. The AI is the hardest. You can continue as many times you want the first 6 fights. If you lose the 7th fight, you have to start againg.
* * * Thanks to @Firkraag for point out “Miyako throws leads into combo, on their wake-up just throw them again”, so just play aggresively using Throws (A+D) then any combo you like. Repeat. Fights becomes very easy * * * .

Win ten consecutive rounds in versus mode.

Research Fanatic
Watch any replay data.
You need to play a Versus match.
At the end of the match, game will let you save a replay. Bottom right of the screen “Save replay data”.
To watch replay you need to go Versus > Replay
Then select the saved Replay from list.
Select Play.
Network
Network Debut
Compete in an online match.
Network > Character Select > Ranked Match or Player Match.
Player or Ranked matches counts.
Training
You Make Me So Proud!
Clear 10 tutorial challenges.
Training > Tutorial. There are 85 Challenges divided into 5 categories: Novice System (11), Begginer System (25), Intermediate System (30), How to Use the System (6) and Battle Strategies Guide (13). The order does not matter, nor the category. Complete 10 any challenges to unlock achievement.
A Firm Foundation is Key

Combo Apprentice
Clear all Mission beginner challenges for 3 characters.
Single Play > Character > Mission. Each character has 4 Beginner Challenges. You need to complete them for 3 different characters.
Combo Practitioner
Clear all Mission intermediate challenges for 3 characters.
Single Play > Character > Mission. Each character has between 5 and 8 Intermediate Challenges. You need to complete them for 3 different characters. You can make things easier by setting up Game Speed to 50% (recommended). While playing challenges Open Menu.
Combo Master
Clear all Mission intermediate challenges for 5 characters.
Single Play > Character > Mission. Each character has between 6 and 8 Intermediate Challenges. You need to complete them for 5 different characters. You can make things easier by setting up Game Speed to 50% (recommended). While playing challenges Open Menu.
Customize
Icon Collector
Purchase 50 customized icons.
Customize > Icons. You can spend your CP here. Each Icon cost 1,000 CP.
Plate Collector
Purchase 20 customized plates.
Customize > Plate. You can spend your CP here. Each Normal plate cost 2,000 CP (Illustration and Pixel Art Plates cost 4,000 CP each).
Title Collector
Purchase 100 customized titles.
Customize > Title > Free Word. You can spend your CP here. Each normal word cost 50 CP.
Big Spender
Spend more than 100,000 CP.
You can get CP by playing in Single Play, Versus, Network and Training modes.
All In Good Taste
Customize the colors of any female character.
Customize > Color Customize.
Select a female character. You can change the colors to your liking or choose a default palette.
All Things In Moderation
Customize the colors of any male character.
Customize > Color Customize.
Select a male character. You can change the colors to your liking or choose a default palette.
Special Title Collector
Obtain 5 special titles as achievement rewards.
Each character has 11 Special Titles tied to achivements: Character Achivement Rate (50, 75 and 100), Play Count (3 and 5), Score Attack Clear Rating S, Time Attack Clear Rating S, Survival Clear Rating S and Mission Clear Rating (B, A and S)
You can check the list on Single Play > Character > Achievement and you see the Titles obtained in Customize > Title > Special.
Character Achievement Rate are tied to other achievements (Story Mode, Score Attack, Time Attack, Mission and Character Achivements). You get Special Titles for completing 50%, 75% and 100%.
Play count 3 and 5. Complete Story, Score Attack, Time Attack and Survival Mode counts for those.
Score Attack Clear Rating S. Complete Score Attack in Easy, Normal and Hard with rank S (the average complete rate for those 3 should be Rank S).
Time Attack Clear Rating S, Complete Time Attack in Easy, Normal and Hard with Rank S (the average complete rate for those 3 should be Rank S).
Survival Clear Rating S. Complete Survival in Easy, Normal and Hard with Rank S (the average complete rate for those 3 should be Rank S).
Mission Clear Rating B. Complete Missions: Special Challenge and Combo Challenge – Beginner.
Mission Clear Rating A. Complete Missions: Combo Challenge – Intermediate.
Missions Clear Rating S. Complete Missions: Combo Challenge – Expert.
Title Plate Collector
Purchase 20 customized title plates.
Customize > Title > Title Plate.
Each Plate cost 1,000, 2,000 and 3,000 CP each.
Voice Collector
Obtain 20 voices as achievement rewards.
Each character has 4 Voice records tied to achivements: Character Select, Character Standby 1, Character Standby 2 and Ending.
You can check the list on Single Play > Character > Achievement and you play Voice records in Gallery > 11. Voice > Character (once they unlocked).
Character Select. You get this one for selecting character (once you confirm selection). Story mode, Versus Mode and Network mode.
Ending. You get this Voice record for completing Story mode wtih a character.
Character standby1 and Character standby2. You get those if you stay idle (dont move controller) before confirmation in character selection (work for both Story and Versus Mode). Wait a few seconds, character start talking, they do this 2 times.
Art Collector
Obtain 30 illustrations as achievement rewards.
Each character has 5 Special Illustrations tied to achivements: Play Count, Story Clear Count, View Scene – EX Special, View Scene – Arc Drive and View Victory Demo
You can check the list on Single Play > Character > Achievement and you see Illustration records in Gallery > 3. Character Selection, 4. Win Illustration, 5. Victory Demo, 6. Ending Illustration and 7. Cut-in Illustration EX Special and 8. Cut-in Illustration Arc Drive > Character (once they unlocked).
Play Count. Complete Story, Score Attack, Time Attack and Survival Mode counts for those.
Story Clear Count. Complete Story mode.
View Scene – EX Special. You get this one after watching specific character perform a EX Special move (even if you are not controlling it).
View Scene – Arc Drive. You get this one after watching specific character perform a Arc Drive move (even if you are not controlling it).
View Victory Demo. You get this one after watching Victory scene for specific character (even if you are not controlling it).

Miscellaneous
Simple Combo
Perform a Rapid Beat. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
Auto-combo by pressing A, B or C.
You’re Wide Open!
Perform a Shield Counter. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
After successfully Shield (D), press A, B, C or B+C.
Let’s End This!
Perform a Forced Release. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
When having at least 1 Magic Circuit bar, press A+B+C to perform a Forced Release and enter to Heat Mode.
Arc Drive Finisher!
Knock out your opponent using an Arc Drive. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
Finish off Opponent with an Arc Drive move. Down, Down+Forward, Forward (236) + B+C when in Heat Mode or have at least 3 Magic Circuit bars.
Last Arc Finisher!
Knock out your opponent using a Last Arc. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
Finish off Opponent with a Last Arc move. A+B+C+D when you have 4 Magic Circuits bars. Default Magic Circuits bars are 3, so you need to lose a round to get an additional bar the next round.
The Strong Prevail
Achieve 5 Perfects. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
I’ll Take You On Any Time!
Go into battle 10 times using a female character. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
Keep Up the Momentum!
Go into battle 10 times using a male character. (Cannot be achieved in Training Mode, Tutorial Mode, or Mission Mode).
Even Weaker Than I Thought
Win a match with a max Moon Icon and a full 400% magic circuit gauge.
The match needs to be set to “2 rounds to win” at least. You need to lose 1 round to access to the 4th Magic Circuit bar. The Moon icon fills while taking or dealing damage. At the end of the match you need to have 4 Magic Circuit bars and Full Moon (Moon Icon filled), so dont perfom any Moon Skill, Moon Drive, EX Special, Arc Drive or Last Arc Move since those require use Moon gauge or Magic Circuits Bars.
Even Time Is An Enemy!
Win by running down the clock.
This can be done in Story, Versus (CPU and Human) and Network. It should come naturally but if you want to boost it, just enter Versus (vs Human) and get it.
